CNIA Membership Guidelines for Registration
CNIA offers several categories of membership to help you personalize your involvement and interaction with the association. The membership categories are regular, institutional, student, associate and regular informatics affiliated members. The current membership year runs from June 1st, 2009 to May 31st, 2010. It is easy to join - just download the rtf membership form below, complete, save and email or mail it along with your fee payment to our Director of Membership Services. Mary Eileen MacPhail.

1. Download the Membership Application Form (90 kb, PDF) complete and save. You can either email it to our Director of Memberships Services, Mary Eileen MacPhail or Print it and Mail it to the address on the form. 2. Register in our Commerce centre to establish a secure account 3. Select the kind of Membership you wish to purchase and follow the instructions. 4. Follow the instructions to go to PayPal and pay using your credit card (American Express, Mastercard, Visa, Discover accepted), direct Bank Transfer or E-check. 5. You can also Download a CNIA Events & Initiatives Brochure 6. Once your membership is established, you are welcome to create an account in our Member's Only Community (if you haven't done so already as a member in previous years). |

Membership Types
Regular - open to any registered nurse, registered psychiatric nurse, certified graduate nurse, registered practical nurse and licensed practical nurses in good standing with their provincial or territorial professional nurses association. If this category is selected, please indicate which College/Association you are currently registered with.
Annual Regular Membership Fee: $56.50 Cdn (includes 13% HST)
Institutional - open to any organization that employs registered nurses, registered psychiatric nurses, certified graduate nurses and licensed practical nurses. If this category selected, please indicate with which organization currently employed (this option provides for up to 5 members from the organization)
Annual Institutional Membership Fee: $169.50 Cdn or $33.90 per member x 5 members (includes 13% HST)
Student - open to any full-time nursing student from an approved nursing education provider. If this category selected, please indicate which school you are currently registered with.
Annual Student Membership Fee: $28.25 Cdn (includes 13% HST)
Associate – open to any other interested party who is not eligible under any of the above categories. Non-nurses may apply for CNIA membership, and the guidelines for participation at CNIA activities are included in the CNIA Membership bylaws.
Annual Associate Membership Fee: $56.50 Cdn (includes 13% HST)
Regular Informatics Affiliated Membership - members in good standing from Existing Jurisdictional Nursing Informatics Association may now obtain a discounted membership rate of $26.25 annually (includes 5% GST) following a written recommendation from their jurisdictional representative.
Annual Affiliate Membership Fee: $28.25 Cdn (includes 13% HST)
